    11. Have you taken one or more of the STOTT PILATES rehabilitation courses listed on this application (below) at Pacific NW Pilates before?
    13. New applicants to Pacific NW Pilates:You must answer ALL questions in the application below. If you have never taken a STOTT PILATES certification course anywhere, you must submit a $25 processing fee with your application before we will begin processing it. If you have already completed at least one official STOTT PILATES course at another location, you do not need to include a letter of reference or a $25 application fee.

      Returning students to Pacific NW Pilates:You must submit an application that provides your current contact information only. You do not need to pay the $25 application fee.

      All applicants:You must provide official documentation of your qualifications as a health professional. (see below) Space will ONLY be reserved in a course upon receipt of your tuition deposit (following acceptance to the program if you are a new student). All Rehab courses require a tuition deposit of $250. Starting in 2010, there will also be a $100 deposit towards your exam for each of the first three modules of the Rehab Certification Program.

      If you are a NEW certification course student at Pacific NW Pilates, you must fill out the remainder of this form AND fill out the anatomy quiz.
    31. Do you think you may need to take STOTT PILATES fitness classes (those available to the general public) in addition to certification classes (teacher training) to gain more physical experience?

    40. Pacific NW Pilates Course Application Policies
    41. 1. Application fees for new students are non-refundable.

      2. All students must read the Policies and Procedures document before submitting their application. This document is updated annually and must be read prior to submitting each application for each course that the student applies for. By submitting your application you are agreeing to all terms contained therein.

      3. A tuition deposit must be paid to reserve your spot in a course. All Level 1 courses require a $500 deposit. For level 2 courses, Injures and Special Populations, and Anatomy, a tuition deposit of $250 is required. For Advanced Mat, a $75 tuition deposit is required.

      4. Tuition must be paid, in full, two weeks (14 days) prior to the course start date to receive the Early Bird 10% discount. The 10% discount is deducted from tuition that remains after the initial deposit is paid.

      5. Students must withdraw or make changes to a course schedule 14 days prior to a course start date in order to receive a full refund. Late cancellations, schedule changes and/or absence from a course will result in forfeiture of the tuition deposit or require make-up time at an additional fee. The remainder of the course tuition and materials fees are non-refundable but may be applied to another course or workshop at Pacific NW Pilates within a 12-month period.

      6. Pacific NW Pilates reserves the right to cancel a course or workshop up to one week (7 days) before the start date and may do so if enrollment is insufficient. We highly recommend that students traveling to attend a course or workshop discuss or confirm scheduling with PNWP staff before making travel arrangements.

      7. In the event of a course cancellation, all paid tuition (excluding the application fee) will be refunded or may be applied to a future course or workshop at Pacific NW Pilates.

      8. Tax for retail items and course supplies will be applied for courses held in states other than Oregon.
    42. Submitting this form indicates that you have read and agree to the application policies above and understand that you must pay a $25 non-refundable application fee and fill out the anatomy quiz if you are a student who has never taken a STOTT PILATES certification course.
